BPPV: The Most Common Cause We Treat

About 70% of vertigo patients I see-including most from Jaipur-have BPPV. This is the benign type, but it’s incredibly disruptive to your life if untreated.

Here’s what happens in detail: Inside your inner ear, there are three semicircular canals. These are fluid-filled tubes that sense your head’s position and movement. They’re like your body’s motion detector. Floating inside these canals is a special fluid with tiny crystals made of calcium carbonate. These crystals help your brain sense gravity and balance. They’re necessary for normal function.

Normally, these crystals stay in their designated area. But sometimes they break loose-after a head injury, after prolonged bed rest, after sudden neck movements, sometimes triggered by stress or extreme heat, sometimes for no obvious reason. Once loose, they float around in the canal. When you move your head, these loose crystals move with the fluid, sending scrambled signals to your brain about your position in space.

Your brain interprets this scrambled information as: “You’re spinning!” Even though you’re standing perfectly still. Your eyes move rapidly back and forth (nystagmus). The room seems to be rotating around you. You might feel nauseous.

Result: Brief, intense spinning sensations-usually 30 seconds to a few minutes-triggered by head movements. Rolling over in bed, looking up, bending down, turning around quickly-these can all trigger it.

The good news: BPPV is highly treatable. Once I diagnose which canal has the loose crystals, I can perform specific maneuvers to move them back where they belong. Success Most people are 90% better within 48 hours of proper treatment.

Repositioning maneuvers for selected BPPV cases

I developed this technique over years of treating anterior canal BPPV-a specific type that’s trickier than the more common posterior canal variant.

The traditional Epley maneuver works great for posterior canal BPPV, but anterior canal cases need a different approach because of the different anatomy and position of the crystals. The Bangalore Maneuver uses a specific sequence of head positions that accounts for the unique anatomy of the anterior canal and uses gravitational forces more effectively to move the crystals.

Instead of relying solely on gravity (which sometimes doesn’t work well for anterior canal crystals), the technique uses gravitational force combined with the specific geometry of the anterior canal to guide the loose crystals back to their proper location. The success rate is higher than traditional maneuvers for anterior canal cases.
