Inside your inner ear are three semicircular canals filled with fluid. These sense your head’s position and movement. Floating in this fluid are tiny crystals made of calcium carbonate (otoliths). These normally stay in a pouch called the utricle.

Sometimes these crystals break loose-after a head injury, after prolonged bed rest, during periods of stress, or sometimes for no clear reason. Once loose, they float around in the semicircular canal. When you move your head, these loose crystals move with the fluid, sending scrambled position signals to your brain. Your brain thinks you’re spinning.

Result: Brief, intense spinning sensations-usually 30 seconds to 2 minutes-triggered by specific head movements (rolling in bed, looking up, bending down).

The good news: BPPV responds incredibly well to proper treatment. Once I identify which canal has the loose crystals, I can perform a specific maneuver to move them back. Success rate: 80-90%.

Repositioning maneuvers for selected BPPV cases

This is a technique I developed specifically for anterior canal BPPV, which is less common but more challenging than posterior canal variants.

The standard Epley maneuver works well for posterior canal BPPV. But anterior canal BPPV requires a different approach. The Bangalore Maneuver uses a specific sequence of head positions personalised to anterior canal anatomy, using gravitational forces more effectively to move the loose crystals back to their proper location.

Success rate for anterior canal BPPV using the Bangalore Maneuver is higher than traditional techniques. I’ve presented this at international conferences. I won the VAI Budapest 2025 award partly for this work. It’s now taught to ENT surgeons across India.
