BPPV: The Most Common Cause We Treat

What actually happens: Inside your inner ear, there are three semicircular canals filled with fluid. Floating in this fluid are tiny crystals made of calcium carbonate (otoliths). These crystals normally stay in a designated pouch.
Sometimes, these crystals break loose-after a head injury, after prolonged bed rest, from stress, sometimes for no clear reason. Once loose, they float around in the canal. When you move your head, the fluid and crystals move, sending scrambled signals to your brain about your position.

Your brain interprets this as: “You’re spinning!” Even though you’re perfectly still. This causes brief, intense spinning-usually 30 seconds to 2 minutes-triggered by head movements (rolling in bed, looking up, bending down, standing quickly).
The brilliant news: BPPV is highly treatable. Once I identify which canal is affected, I can do a specific maneuver to move the crystals back where they belong. Success rate: 80-90% with proper technique.
Repositioning maneuvers for selected BPPV cases
This is a technique I developed specifically for anterior canal BPPV, which is trickier than the more common posterior canal variant.
The traditional Epley maneuver works well for posterior canal BPPV. But when the loose crystals are in the anterior canal, you need a different approach because the anatomy is different. The Bangalore Maneuver uses a specific sequence of head positions that accounts for the unique anatomy of the anterior canal and uses gravitational forces more effectively.
The success rate for anterior canal BPPV using the Bangalore Maneuver is higher than traditional techniques. I’ve presented this at international conferences. I won the VAI Budapest 2025 award partly for this work. It’s now being taught to ENT surgeons across India.
