A canalith repositioning procedure uses gravity and planned head positions to guide those crystals out of the canal. The aim is to reduce the false spinning signal, not to treat every possible cause of dizziness.

When it is used

This procedure is most often used for BPPV treatment. The doctor first looks for a typical history, positional trigger and eye-movement pattern. A test such as the Dix-Hallpike test or a roll test may help identify the canal and side.

Patients may describe “bed pe palatne se chakkar,” “looking up causes spinning,” “bending down gives vertigo,” or “room spins for a few seconds.” These words are useful, but the maneuver choice still depends on the clinical pattern.

Common examples

The Epley maneuver is a common canalith repositioning procedure for posterior canal BPPV. The BBQ roll maneuver is used for selected horizontal canal BPPV patterns. Other maneuvers may be chosen depending on canal, side, nystagmus direction, neck comfort and balance safety.

These details help decide whether the procedure is appropriate, modified or delayed.

Bring old prescriptions, previous maneuver details, VNG or hearing reports if available, and a note of which side triggers the spinning. Also mention whether symptoms are strongest while lying down, sitting up, turning right, turning left or looking upward. This makes the visit less guesswork-based.
